360° Feedback Survey

Dear Colleague:

This questionnaire has been specifically designed to look at those leadership behaviours, which are considered to be important in helping to transform Marsh. The responses you give will be used in conjunction with others, to produce a feedback report to the individual involved. Feedback from the report can be invaluable in strengthening an individual's leadership behaviours through the enhanced understanding of their strengths and development needs.

For the results of the feedback process to be useful, it is important that people provide a truly accurate picture of how they see things. Please take the time to answer each question carefully and honestly with regard to both frequency and importance. To respond to an item, fill in the circle on each scale that best corresponds to your opinion. This is not a test, and there are no right or wrong answers. If a particular question does not seem to apply to our situation, fill in the circle on the scale corresponding to "Not Applicable." Use the definitions below when thinking about the appropriate answer for each statement.

Behavior Frequency: 7 = Almost Always   5 = Often   3 = Sometimes   1 = Seldom   N/A = Not Applicable or Not Observed
Importance: 7 = Critical   5 = High Importance   3 = Low Importance   1 = Neutral   N/A = Not Applicable or Not Observed

Your responses are confidential. Individual ratings will not be disclosed to the individual being rated except for the manager's rating.

Please take the time to complete the survey as soon as possible for all colleagues listed in the drop down box after the login screen; it should take about 10-15 minutes for each participant depending on how well you know the colleague.

To complete the survey for your colleague (or colleagues), please complete the following steps:

  1. Click the Arrow below for the login screen..

  2. Enter your external email address in lowercase and put the password listed in your email, then click login.

  3. A list of the colleagues that requested your feedback will be displayed. Highlight the participant's name from the drop down box, and then click Submit.

  4. A survey page will be displayed for the colleague you selected. Complete the survey by making sure that you click one number or NA in response to each question. There are three open ended questions at the end of the survey please make sure you complete these as well.

  5. When you are done with the survey, please click on the "Submit Survey" button once and wait for the survey to be processed. DO NOT CLICK ON THE SUBMIT BUTTON MORE THAN ONCE.

  6. Your Survey input for that individual colleague is now complete.

  7. Survey Submitted page will be displayed. If you have multiple colleagues to provide feedback for, select "Another Participant" and click submit. If you are finished, select "Log Out" and click Submit. Complete as many colleagues' surveys as you want at one time. However, you must fully complete and submit each survey in order for it to be accepted.
